Payment Policy

We request that you provide us with your current insurance card.  The card should be readable.  It should have the claims mailing address on it and we will need your identification number for the policy.  If you have a policy that restricts which providers you see we encourage you to call them before seeing us.  We participate in most major insurance plans.

All co-payments are due at the time of the appointment you can find this amount on your insurance card.

If you do not have insurance coverage we will require payment at the time of service.  If this is a hardship for you, please call the office and set up a payment plan prior to your appointment.  We do accept all major credit cards.

Medicare and most supplemental plans do not cover the refraction fee, this charge will be collected at the time of your visit.

Optical Purchases

Our optical department requires a 30% non-refundable deposit prior to ordering frames and or lenses.  The balance is due at the time the glasses are delivered to you.  We do not participate in any private vision/optical insurance plans.   However, upon request we will provide you with an itemized bill you can submit for reimbursement from your insurance.
